Introduction
ERSCP is required to undertake a local child safeguarding practice review where:
- abuse or neglect of a child is known or suspected
- a child has died or been seriously harmed
- there is cause for concern as to the way in which the local authority, safeguarding partners or other relevant persons have worked together to safeguard the child.
Individual case review panel
To carry out a child safeguarding practice review, ERSCP will establish an individual case review panel. ERSCP will also make sure that any learning is shared and embedded into practice.
Criteria
A review may be considered when there are concerns about the way in which local professionals and services worked together to safeguard and promote the welfare of children (including inter-agency and inter-disciplinary working), and a child:
- dies
- is seriously harmed
- sustains a potentially life-threatening injury or a serious and permanent impairment of physical and mental health and development through abuse or neglect
- has been seriously harmed as a result of being subjected to sexual abuse
- has been seriously harmed following a violent assault perpetrated by another child or an adult.
A review might also be considered when:
- a parent has been murdered and a domestic homicide review is being initiated under the Domestic Violence Act 2004
